Search Criteria by Vision Plan

When verifying eligibility or pulling authorizations in SpexFetch, each vision plan accepts specific patient or policyholder identifiers. Below is a breakdown of what you can use — and remember, each method is an “either/or,” not a combination.

👍 General Rule of Thumb: Provide More Information for Better Results

The more information you provide the system, the better and more bountiful your search results will be.
While it’s tempting to only enter a member ID, this can often lead to incomplete or incorrect results. Here’s why:
  • Patients frequently give expired ID numbers from former employers or previous years
  • ID numbers may be mistyped or copied from outdated cards
  • A single ID will often only return one plan, even if the patient has multiple plans (e.g., one through themselves and another through a spouse)
What to enter during a search:
  • First Name
  • Date of Birth
  • Member ID (if available)

Providing all available details allows the system to search across more combinations, giving you more accurate, complete, and faster results across all participating vision payers.

Some Plans Require a Member ID

This is a limitation imposed by the vision payer, and it cannot be bypassed by SpexFetch.
If a patient doesn’t know their Member ID, you’ll need to request it directly from them.
Without it, no results may be returned, even if all other information is accurate.

❌ What Happens If You Don’t Use the Right Search Criteria?

If you attempt to run a search without the required fields for a specific vision plan, SpexFetch will return an error message notifying you that your search criteria is incomplete or invalid.

Pro Tip: Always double-check the name spelling, date of birth, and whether you're searching for the policyholder or dependent. A small detail can affect your results.

Carrier-Specific Search Criteria

💎 VSP (Vision Service Plan)

You must use the primary policyholder's information to return both the policyholder and any dependents.
Search using one of the following:
  • First Name, Last Name, and Date of Birth (of the policyholder)
  • Full Member ID (of the policyholder)
  • First Name, Last Name, Date of Birth, and last 4 of SSN
    • 🔐 The last 4 of SSN goes in the Member ID field
      Especially helpful if search for a patient with a common name and identical DOB

🟢 EyeMed

Search using either:
  • First Name, Last Name, and Date of Birth
  • Member ID

🔷 Versant (Davis Vision & Superior Vision)

Ssearch using either:
  • First Name, Last Name, and Date of Birth
  • Date of Birth and Member ID

🟣 Spectera

Search using either:
  • First Name, Last Name, and Date of Birth
  • Member ID

🟠 March Vision

Search using either:
  • First Name, Last Name, and Date of Birth
  • Member ID
